Why tire and wheel-end failures deserve fleet-level attention
A failed steer tire can change direction instantly. A failed drive or trailer tire can damage air lines, body panels, mudflaps, wiring, cargo equipment, or adjacent tires. A wheel-end failure can create fire, bearing damage, wheel separation, or an extended roadside event. These failures are rarely “just a tire.” They expose drivers, technicians, and the public to secondary risk long after the initial component fails.
The strongest prevention strategy starts with routine condition monitoring and clear removal criteria. Drivers need a realistic inspection method; technicians need accurate pressure, torque, and bearing procedures; managers need data that reveals chronic underinflation, alignment wear, repeated seal leaks, and problem wheel positions.
Inflation is a load-carrying issue
A tire carries load through air pressure. Underinflation increases deflection and heat, can accelerate casing damage, and changes how paired dual tires share weight. Overinflation can reduce the desired footprint and contribute to irregular wear. The correct pressure is not whatever number looks round on a gauge; fleets should establish pressure targets based on tire, axle load, application, manufacturer guidance, and operating policy.
Check pressure cold whenever possible and use accurate gauges. A visual kick test cannot reliably identify moderate pressure loss. If one tire repeatedly needs air, treat that as a leak investigation. Valve cores, stems, rims, punctures, bead areas, and tire-pressure-monitoring hardware can all be sources. Continually topping off a leaking tire is not preventive maintenance.
Tread, sidewalls, and irregular wear tell a story
Federal rules require at least 4/32 inch tread depth in major grooves on front tires and at least 2/32 inch on other tires. A fleet should not use those minimums as its only decision point. Cuts, bulges, exposed material, penetrations, chunking, severe weather cracking, flat spotting, and abnormal wear can require action even when depth is adequate.
Wear pattern is diagnostic information. Feathering, one-sided shoulder wear, cupping, rapid center wear, and diagonal wear may point to alignment, inflation, suspension, balance, shock, bearing, or application issues. Photograph and code repeat patterns. Replacing tires without correcting the cause turns the tire budget into a symptom-management program.
Dual assemblies need equal partners
Dual tires should be compatible in size and condition and should not contact one another. A significant difference in inflation or rolling diameter can make one tire carry more of the load and generate more heat. Debris trapped between duals can cut sidewalls or create friction. Inspect the inner tire, inner sidewalls, valve access, spacing, and anything wedged between the pair.
When one dual fails, inspect its mate closely. It may have carried overload, absorbed debris, or been exposed to heat during the event. Simply replacing the visibly failed tire can leave a weakened companion in service. Fleets should have a defined inspection standard for the remaining tire after a dual failure.
Wheel fasteners, rims, and mounting surfaces
Rust trails around a fastener, shiny movement marks, elongated holes, cracked rims, damaged studs, or missing nuts deserve immediate investigation. Proper wheel installation depends on clean and compatible components, correct seating, correct fasteners, and manufacturer-specified torque procedures. An impact wrench is a tool for removal and controlled installation support; it is not a calibrated proof of final torque.
After wheel service, follow the wheel or vehicle manufacturer’s guidance for torque verification and any required recheck. Train personnel to recognize stud-piloted and hub-piloted systems and the differences in hardware and lubrication instructions. Incorrect lubricant placement or mixed components can change clamp load and create failures.
Hub seals, bearings, and heat checks
Oil or grease on a wheel, brake components, or inside tire is a warning. A leaking hub seal can reduce lubricant and contaminate brakes. Bearing condition, end play, lubrication level, seal installation, and vent condition all matter. Drivers should report fresh leakage, burnt odor, smoke, or an unusually hot hub; technicians should identify the source before cleaning away the evidence.
Temperature comparisons can be useful when performed safely and consistently, but they do not replace mechanical inspection. A hot wheel end can result from brakes, bearings, inflation, or loading. A cold brake in a set can also be suspicious if it indicates that component is not doing its share of stopping.
A supervisor’s field audit: what to look for
A safety rule becomes reliable only when supervisors can observe it in normal work. Conduct short field audits in yards, at terminals, during ride-alongs, or through appropriate video review. Look for the behavior the policy is supposed to produce, then ask the employee to explain the reason behind it. A person who understands the hazard is more likely to make the correct decision when the exact situation is not written in a checklist.
Record both strengths and gaps. If several employees make the same mistake, examine training, equipment, work design, or expectations before treating it as a collection of individual failures. Repeated behavior usually has a repeated cause. Correcting the system is more durable than issuing the same reminder every month.
Training questions that reveal real understanding
Instead of asking only whether an employee completed training, use scenario questions: What would make you stop the job? What change would make this condition unsafe? Who do you call when you are unsure? What evidence would you put in a defect or incident report? What is the safest alternative if the planned method cannot be used? These questions expose whether the employee can apply the rule under pressure.
Refresher training should use actual fleet events, photographs, inspection findings, near misses, and equipment examples. Remove names when appropriate and focus on the decision points. People learn faster from a familiar tractor, trailer, route, dock, or component than from a generic slide that does not resemble their work.
Documentation should support safety, not bury it
Keep records that help the next person make a decision. Use consistent defect codes, clear descriptions, photographs when useful, unit and component location, dates, repair disposition, and responsible personnel. Avoid turning documentation into so many fields that employees rush through it. The best record is complete enough to reconstruct the condition and simple enough that people will actually use it.
Finally, review the data. A folder full of completed forms is not a safety program. Trend defects, near misses, roadside events, coaching observations, and repeat repairs. Share the lessons with drivers and technicians so they can see that reporting produces action. That feedback is what keeps a safety system alive.

Tires and wheel ends carry every pound the fleet moves. The safest fleets manage them as a system of pressure, load, heat, fastening, lubrication, and inspection—not as consumables changed only when tread is gone. The practical standard is to build enough margin that a single missed cue, unexpected traffic move, equipment defect, or weather change does not immediately become an emergency. Drivers, technicians, dispatchers, and supervisors all influence that margin.
